Causes of Liver Cirrhosis

Liver cirrhosis can result from various liver diseases and conditions, including:

  • Chronic Alcohol Abuse: Long-term excessive alcohol consumption is a leading cause of cirrhosis. Alcohol damages liver cells, leading to inflammation and scarring.
  • Hepatitis B and C: Chronic infection with these viruses can cause liver inflammation, eventually leading to cirrhosis. Hepatitis C is a particularly common cause.
  • Non-Alcoholic Fatty Liver Disease (NAFLD): This condition, often associated with obesity, diabetes, and metabolic syndrome, involves fat accumulation in the liver, leading to inflammation and scarring.
  • Autoimmune Hepatitis: This occurs when the body's immune system attacks liver cells, causing inflammation and damage.
  • Genetic Diseases: Conditions like hemochromatosis (excess iron accumulation) and Wilson's disease (excess copper accumulation) can cause liver damage.
  • Biliary Diseases: Diseases such as primary biliary cholangitis and primary sclerosing cholangitis, which affect the bile ducts, can lead to cirrhosis.

Symptoms of Liver Cirrhosis

In the early stages, liver cirrhosis may be asymptomatic. As the disease progresses, symptoms become more apparent and severe:

  • Fatigue: Persistent tiredness is a common early symptom.
  • Jaundice: Yellowing of the skin and eyes due to increased bilirubin levels.
  • Itchy Skin: Caused by bile products deposited in the skin.
  • Easy Bruising and Bleeding: Due to reduced production of clotting factors by the liver.
  • Swelling: Edema (swelling in the legs and ankles) and ascites (fluid accumulation in the abdomen).
  • Nausea and Loss of Appetite: Often accompanied by weight loss.
  • Confusion and Difficulty Thinking: Known as hepatic encephalopathy, this occurs due to the buildup of toxins in the brain.
  • Spider Angiomas: Small, spider-like blood vessels visible on the skin.

Stages of Liver Cirrhosis

Liver cirrhosis is typically classified into four stages:

  1. Stage 1: Compensated Cirrhosis
  2. In this stage, the liver is heavily scarred but still able to perform most of its essential functions. Symptoms may be mild or absent.
  3. Stage 2: Compensated Cirrhosis with Varices
  4. The liver begins to develop increased pressure in the portal vein, leading to the formation of varices (enlarged blood vessels) in the oesophagus or stomach.
  5. Stage 3: Decompensated Cirrhosis
  6. Liver function is significantly impaired, and symptoms become more severe. Complications like ascites, variceal bleeding, and hepatic encephalopathy are common.
  7. Stage 4: End-Stage Liver Disease (ESLD)
  8. The liver is extensively scarred and cannot perform its functions adequately. Liver transplantation may be necessary at this stage.

Liver Transplant Surgery
  • Blood Tests: To assess liver function and detect markers of liver damage.
  • Imaging Tests: Ultrasound, CT scans, and MRI to visualize the liver and assess the extent of damage.
  • Liver Biopsy: A sample of liver tissue is examined to confirm the diagnosis and determine the severity of cirrhosis.
  • Elastography: A specialized ultrasound technique to measure liver stiffness, indicative of scarring.

Treatment focuses on managing the underlying cause, preventing further liver damage, and addressing complications:

  • Lifestyle Changes: Avoiding alcohol, maintaining a healthy weight, and managing underlying conditions like diabetes.
  • Medications: To treat hepatitis, control symptoms, and manage complications.
  • Endoscopic Treatments: To treat varices and prevent bleeding.
  • Liver Transplant: In cases of end-stage liver disease, a liver transplant may be the only option.
